Are Irregular Periods Affecting Your Chances of Pregnancy?

Irregular menstrual cycles are often ignored until a woman starts planning a pregnancy. As a Gynaecologist and Infertility Specialist, I frequently meet women who believe irregular periods are normal because they have experienced them for years. While occasional changes can occur, persistently irregular cycles may indicate underlying hormonal or reproductive conditions that deserve evaluation. Understanding the reason behind irregular periods is an important step towards protecting your reproductive health and improving fertility when pregnancy is planned.

The challenge with irregular periods is that they rarely cause significant day-to-day discomfort, so many women accommodate them as part of their normal experience. It is only when pregnancy does not happen that the connection between cycle irregularity and ovulation is made. Why Do Periods Become Irregular?

Several conditions can affect the menstrual cycle, including polycystic ovary syndrome (PCOS), thyroid disorders, hormonal imbalances, excessive stress, sudden weight changes, and other medical conditions. Identifying the underlying cause allows treatment to be tailored appropriately rather than simply managing symptoms.

PCOS is the single most common cause of irregular periods I see in my clinic, and it is also one of the most treatable when approached correctly. A woman with PCOS may have periods that come every two, three, or even six months, or may go through extended periods of no cycle at all. The good news is that with the right combination of lifestyle support and targeted treatment, many women with PCOS achieve regular ovulation and successful pregnancy without needing advanced fertility treatment. Thyroid disorders are the other frequently missed cause of cycle irregularity, because the symptoms can be subtle and are often attributed to other factors. A simple blood test can identify thyroid dysfunction, and treatment almost always restores normal cycle regularity.

Ovulation Plays an Important Role

Women with irregular cycles may not ovulate regularly, making conception more difficult. Through detailed history, examination, ultrasound, and appropriate investigations, we can determine whether ovulation is occurring and recommend suitable treatment options.

A key investigation I use is a monitored follicular study using pelvic ultrasound, which allows us to track whether a follicle is developing and releasing an egg during the cycle. This provides far more actionable information than a single blood test and helps us determine whether ovulation induction is needed and how the body is likely to respond to it.

Early Assessment Makes a Difference

Many women delay seeking medical advice because they assume irregular periods will eventually settle on their own. Early evaluation can help identify treatable conditions before they begin affecting fertility or overall health.

The reality is that waiting is rarely the right strategy when cycles have been irregular for more than a few months. The underlying condition does not usually resolve on its own, and in some cases, such as untreated PCOS or thyroid disease, it can gradually worsen. Early assessment, by contrast, often reveals a straightforward diagnosis and a clear treatment path.
